Cancer is the leading cause of death worldwide. It is defined as the uncontrolled growth of abnormal cells in the body that affects healthy cells, have deleterious effects on vital organ functions and eventually death. New and potent anticancer medications are urgently required. There are many different sources of alkaloids that may have anticancer properties. Several of them have the potential to be local anesthetics but their application is primarily restricted to clinical uses based on the patients need. Ongoing efforts by scientists persist in the development of more complex semisynthetic and synthetic alkaloids obtained from natural alkaloids. These alkaloids may exhibit intriguing activity for medicinal, pharmacological and many other beneficial qualities. Consequently, the present review article outlines the modes of action of naturally occurring alkaloids that may have anticancer properties, select synthetic methods and SARs of these privileged motifs. Current knowledge in this sector is expected to be beneficial for scientists and students alike.
